Family-Based Wraparound Services for High-Need Youth

The Short-Term Adjunctive Youth and Family Engagement (SAYFE) is a family-focused, individualized, and strengths-based wraparound program. We serve youth (ages 6-18) and their families throughout San Mateo County, where they feel most supported and safe – in their homes, schools, and communities. Our wraparound services are tailored to meet each youth’s specific mental health, educational, social, and cultural needs.

SAYFE helps the highest risk children and youth living in San Mateo County achieve independence, stability, and wellness within the context of their family, community, and culture. Children and youth are referred to the program with a primary clinician in place from either a school setting (outpatient classroom or Therapeutic Day School), or Behavioral Health and Recovery Services (BHRS) Regional County Clinic.

SAYFE supports, augments, and extends an existing treatment plan, with our multidisciplinary team of providers. Our team communicates and collaborates with the primary clinician on a regular basis to ensure continuity of care. We also provide intensive home-based support and Child & Family Team Meetings (CFTs).

Services Offered

Family Therapy

Delivered in the home, school, or community to promote engagement, communication, and stabilization.

Case Management

Intensive support to coordinate care and connect families with needed resources.

24/7 Crisis Intervention

Immediate response and stabilization to help manage acute situations and prevent escalation.

Caregiver Peer Support

Peer advocacy and support for parents/caregivers to enhance engagement and empowerment.

Continuity of Care

Primary county clinician (school-based or BHRS) remains actively involved throughout SAYFE engagement for seamless case coordination.

Who We Help

The program serves up to 45 youth ages 6 to 21 and their families at a time. Participants must have high acuity in the following areas:

  • Severe emotional/behavioral needs and/or complex system involvement
  • Risk of out-of-home placement due to factors such as recurrent psychiatric hospitalizations, involvement in juvenile justice or child welfare, or repeated school exclusion

Referral Process

Referrals must originate from school-based (Outpatient Classroom, Therapeutic Day School) or BHRS County clinicians, who continue as the primary providers during and after SAYFE involvement.

Program Outcomes

  • Reduce frequency and duration of psychiatric hospitalizations, juvenile justice system contact, and reliance on out-of-home placements
  • Improve school attendance and academic engagement
  • Enhance participation in community and therapeutic activities
  • Foster measurable positive change in family functioning and treatment engagement
  • Client/family satisfaction and progress measured at discharge via survey and team-completed summary
  • Progress and outcomes are reviewed in monthly multidisciplinary provider meetings involving the referring county/school clinicians
