Family-Based Wraparound Services for High-Need Youth

Turning Point Child and Youth (TPCY) provides intensive, community- and family-based wraparound services for high-need children and young adults ages 6 to 21 residing in San Mateo County. The program is structured to support youth who are impacted by severe emotional and behavioral challenges at risk of psychiatric hospitalization or higher levels of out-of-home care and their families. Turning Point is also meant to serve youth transitioning back into the community from more restrictive placements.

Turning Point is well suited for youth and families with complex behavioral health and system-involvement needs—when traditional outpatient or school-based resources are not sufficient to maintain stability in the least restrictive setting. The program’s wraparound and multidisciplinary approach prioritizes safety, resilience, and sustainable community integration.

Comprehensive clinical assessment and collaborative treatment planning
Intensive case management and care coordination
Individual and family psychotherapy
Monthly child and family team (CFT) meetings
Collateral and rehabilitative services
Intensive home-based support
24/7 crisis intervention and stabilization
Integrated substance use and co-occurring disorders services
Psychiatric evaluation, medication management, and ongoing monitoring if needed

Who We Help

Youth ages 6 to 21 who are residents of San Mateo County and are identified as having significant behavioral health needs and their families. These youth may be dually diagnosed with significant behavioral health needs and, substance use, or developmental conditions.

Context that may exist for these youth includes:

  • History of multiple psychiatric emergency interventions, frequent or prolonged hospitalizations, or a risk of out-of-home placement
  • May include youth returning from residential care or those involved with juvenile justice or child welfare systems
  • Youth experiencing “first break” psychosis or recent diagnosis of a psychotic disorder

Turning Point Program Outcomes

Turning Point aims to:

  • Prevent escalation to higher levels of care (e.g., psychiatric hospitalization, residential placements, juvenile detention), and support successful step-down to sustainable home- or community-based settings.
  • Support stability, family permanence, and wellness within the youth’s natural environment and cultural context.
  • Document progress toward individualized treatment goals.

The Turning Point program at Edgewood uses Welligent EHR to track clinical services and progress including:

  • Special Incident Reports and crisis line utilization
  • Key Event Tracking for hospitalizations, juvenile holds, and higher-level placements

Provider Referrals

  • Referrals are made through the Inter-Agency Placement Review Committee held weekly through San Mateo County BRHS.
  • Referring providers are encouraged to remain actively involved to ensure care continuity and comprehensive cross-system collaboration.
