Transitional Housing and Independent Living Support for Former Foster Youth
Edgewood’s Transitional Housing Plus (THP+) program provides safe, stable housing and comprehensive independent living skills support for transition-age young adults who have exited foster care or juvenile probation after their 18th birthday. The program offers structured, supportive housing for up to 36 cumulative months or until age 25 (whichever comes first).
Rent subsidies and regular home inspections to ensure safe, stable living conditions.
Case Management & Life Skills Training
Ongoing support with budgeting, employment readiness, education/career planning, healthcare navigation, and daily living skills development.
Goal-Oriented Support
Assistance to achieve educational milestones (GED/high school diploma), employment, health, wellness, and parenting goals.
Individualized Transition Planning
Each participant receives a tailored transition plan built around their self-identified needs and goals, with regular progress monitoring and skill building.
Holistic Care Coordination
Links to clinical, behavioral health, and community-based services as needed for whole-person support.
Who We Help
Youth aged 18 to 25 (must exit the program by age 25 or after 36 cumulative months) who had a foster care placement order in California at age 17.5 and emancipated between ages 18 and 21.
Includes youth formerly supervised by juvenile probation.
Must reside within a 2-hour radius of the City and County of San Francisco.
Youth at risk of homelessness or housing instability after foster care; clients with outstanding needs for life skills, vocational, educational, or social/emotional stabilization and parenting youth.
Referral Process
Providers are encouraged to refer eligible young adults exiting foster care who would benefit from intensive housing support and individualized skill-building to promote a successful transition to adulthood.

Program Outcomes
THP+ aims to:
Prevent homelessness during and after the program
Increase participants’ self-sufficiency and life skills (target: 75% skill improvement as measured by the Ansell Casey Life Skills Assessment)
Ensure completion of high school/GED for all participants
Support progress toward vocational, employment, wellness, and/or parenting goals
Maintain high rates of participant satisfaction (≥90%)
