The Edgewood Enhanced Care Management (ECM) Team provides person-centered support to Medi-Cal recipients enrolled in the San Francisco Health Plan, serving youth up to age 21 (and current or former foster youth up to age 25). We work in tandem with families to find, access, and coordinate community supports. Referrals can come from any individual member, medical provider, or community partner.

The Edgewood Enhanced Care Management Team is here to support families in finding and accessing community services and programs that help with health, mental health, housing, food security, legal, educational, vocational, and social/recreational goals.

Get Started

To be eligible for ECM, youth must be enrolled in a Medi-Cal managed care plan — San Francisco Health Plan — and meet one or more of the following criteria:

  • Up to age 21, experiencing or at risk of homelessness
  • Up to age 25, if a current or former foster youth
  • Up to age 21, with behavioral health needs
  • Up to age 21, experiencing frequent hospitalization
  • Pregnant and postpartum individuals

How We Help

Edgewood works with enrolled members to assess and create care plans focused on their goals. As a person-centered program, our ECM Resource Specialists work alongside members to find and access social service resources, and coordinate services with providers such as doctors, medical appointments, housing, transportation, and other care teams.

What We Offer

The Enhanced Care Management (ECM) Resource Specialist will coordinate meetings with families to identify the needs and services that will best support them, and then help families connect with those services. Through these support services, families can navigate and achieve their goals in a supportive environment.

Access & Referrals

Parents and caregivers can call us directly — no referral needed. We also accept referrals from: Schools, Outpatient providers, Hospitals & emergency services.
