Transitional Age Youth Full Service Partnership

Intensive, Community-Based Mental Health Services for Emerging Adults in San Mateo County

Edgewood’s Transitional Age Youth (TAY) Full Service Partnership program delivers intensive, wraparound-informed mental healthcare services to transitional age youth ages 16 to 25 with complex clinical and psychosocial needs.

The program is specifically designed to reach young people with the highest acuity outside of inpatient or residential care. These may include those with significant trauma histories, serious mental health diagnoses, co-occurring substance use, risk of homelessness, justice involvement, and disrupted functioning across life domains.

The treatment team may include a case manager, licensed therapist, behavioral health coach, optional family partner, and psychiatric providers. Services are delivered flexibly in environments that best support the client: their current living situation, hospital or locked settings, community locations, schools, our offices, or via telehealth.

Who We Help

San Mateo County youth or young adults ages 16 to 25 with complex trauma, serious behavioral health disorders, or co-occurring diagnoses.

These youth and young adults are often at high risk of homelessness, criminal justice or have child welfare involvement, frequent hospitalizations, or significant functional impairment.

Referral Process

  • Most referrals are coordinated via the San Mateo County BHRS Youth Transition Assessment Committee (YTAC)
  • Referrals may be submitted by county or public agencies, community partners, or other Edgewood programs

Transitional Age Youth Full Service Partnership (TAY FSP) supports clients to achieve stability, wellness, and independence across multiple domains:

  • Mental and behavioral health symptom reduction
  • Stable housing and reduced risk of homelessness
  • Increased educational/vocational engagement and success
  • Social connection and natural support network development
  • Reduced rates of hospitalization, incarceration, or justice system contact
  • Enhanced adaptive and coping skills
  • Clinical and program team collaborates with referral sources to ensure effective transitions and continuity of care
